How do I use Microsoft Forms to create surveys and quizzes?

Resolution

Microsoft Forms have some limitations. You can create up to 400 forms/quizzes and polls. This form limit includes those in the recycle bin. Each form has a 200 question limit and can receive up to 5,000,000 responses per form.
  1. Log into Microsoft 365.
  2. Click and select Forms.
    Note: If this is the first time you have used Forms, you may be given a walk through on how to use the Microsoft Forms. The next steps assume you have already done this and needing to create a new form.
  3. Click New Form to start a new form.
  4. Add a title and description for your form.
  5. Select the type of question you want (e.g., choice, text, rating).
  6. Enter the question text and any possible answers if applicable.
  7. Click Add Question to insert your next question. Repeat steps 5 through 7 to add more questions as needed.
  8. Click Style at the top to customize the appearance of your form.
  9. Once you finish your form, click Preview to see how your form looks.

To share the form, click Collect Responses, and choose your sharing options (link, email, QR code). You can collect responses and view results by clicking on View Responses.
