How do I embed a Formstack form on a WKU webpage?
Environment
- Formstack Suite platform (currently focusing on Forms module)
Cause
Formstack forms must be embedded on WKU webpages using the approved Formstack Form Embed Component. This helps ensure the form displays consistently and uses the correct embed format.
Resolution
Before embedding the form, apply the
WKU Embeddable theme in Formstack. This theme does not include the WKU header or footer and is intended for forms embedded within WKU webpages. See the article,

Copy the Form URL:
- In Formstack, open form you want to embed.
- Go to Settings tab.
- Locate Form URL.
- Copy only form name from URL (after /forms/).
- your_form_name
- https://wku.formstack.com/forms/your_form_name
Add Component to a Page:
- In Web CMS, go to page that needs form displayed.
- Check out page.
- Place your cursor where form should appear.
- Click Insert Component in WYSIWYG editor toolbar.
- Select Formstack Form Embed component and click Insert.
- Paste your URL in Form URL field.
- Save component and then the page to preview.
- Publish page when ready.
