New Employee Onboarding Form

Utilize this request to submit your new employee biographical data information.

• Please attach the following completed and signed forms to this request:
  1. Warren County Schools Payroll Tax Withholding Form
  2. Federal Payroll Tax Withholding Form (W 4)
  3. Kentucky Payroll Tax Withholding Form (K 4)
  4. Direct Deposit Authorization Form (PDF)
     Note: The direct deposit form requires a voided check or statement from your bank confirming the routing/account numbers.

If you are unable to complete these forms online, they will be provided at your new hire appointment.

During your new hire appointment, you will provide identification for section 2 of the I-9. A list of acceptable documents can be found here.
