How do I create a new email using a form (template) in Outlook?

Environment

  • Windows 10+
    • Outlook M365

Resolution

  1. Click Developer tab.
    Note: Developer tab is not visible by default and must be enabled.
  2. Click Choose Form.
    Note: You must have already created and published a form (template) to select.
  3. Select Personal Forms Library in the Look In dropdown.
  4. Double click on the name of your form (template).
  5. The form will pull up as a new email, and you can make any changes or adjustments before sending like any other email.
